Medic Alert Benefit
This benefit offers members with conditions requiring special medical treatment a free first-year membership in this life-saving program, which includes the cost of a stainless steel identification bracelet or necklace. In an emergency, information about their special needs will be instantly available to caregivers and emergency personnel.
Qualification Criteria:
Applicant must be a member of Degree of Honor, which means he/she is the primary insured on a life insurance certificate or on a spousal rider, or an annuitant. In order to apply for this benefit, please click on the link below to complete a fillable PDF of the form required by Medic Alert.
Submit the form through one of these two ways:
1. Save an electronic copy of the form to your computer and attach it to an email addressed to: memberbenefits@catholicfinanciallife.org

*If you would prefer to upgrade to a higher quality bracelet or necklace, other than the stainless steel variety included with the basic membership, please submit your form by mail and include a check for the difference in cost .
